Referral Letter

Referral letter requirements

A complete referral letter is essential for a smooth admission. Below you'll find the elements the letter should contain.

A clear referral to specialised mental health care: Hillhouse Clinics, 8 Klein Constantia Rd, Constantia, Cape Town, 7708, South Africa
A reasoned choice for specialised mental health care including: suspected DSM diagnosis, severity, risks, complexity, current medication and course of symptoms
A referral dated no more than 9 months prior to treatment and no later than the date of the first appointment
The name, role and AGB code of the referring professional
A signature and/or practice stamp of the referrer
The referral letter must be drafted in English

Important: For effective treatment, full insight into your patient's medical history is crucial. We kindly ask you to provide as much relevant information as possible.
